Case Manager Social Worker Family Medicine
Memorial Medical Center Case Manager will assist manage patients' transition through the healthcare system and connect patients and families with necessary resources to promote health and well-being.
Accountable for accurate resource utilization, facilitation of improvement in quality care for assigned caseload, and completes timely review and completion of necessary documentation.
Provide services to patients throughout the lifespan including all age groups.
Engagement in care coordination and inter-professional consultation meetings and development of social work programs in a teaching setting.
